In "With Ensigns Flying" which I am reading now (about destroyer actions) his nickname of "The Mad Admiral" is mentioned. Why did he get this nickname?

If Ruthless, Unforgiving, Unknowable: Admiral Sir Philip Vian pt 2 can be believed, it came from Admiral Cunningham, but it doesn't explain what about Vian's character or actions earned the nickname.
