Uncompleted Army forms and standard internal forms for Casualty Prisoner of War and other War Office departments are hard to come by. This is unusual in that it is from German clerks to British clerks found on a file in WO361. The back of the form was used for a file note on a file relating to prisoners of the Japanese.

I really like all the Army Forms and have a lot of them (used and unused) but I've never seen one like the first that basically crosses the front line - does it have a number at the top right? "AF....." or "Army Form ...."

This is handy for checking them

The form in post 1 seems to have a reference I did not photograph completely ( back in September 2010 ). It is on file WO361/197 which deals with casualties concerning the Lisbon Maru. The reference begins: " M13053" then could be 3 or 1 then could be 5 47 ? " Enlargement here:

Internal Cas. Pw. form for tracing men listed by Tokyo Cables, i.e. from the Japanese Prisoner of War Information Bureau, and the SB reference, which is the Swiss cable reference. These SB references could become very tangled, especially when some went missing or were not received in full by wireless:

A few stamps from Foreign Office files: the "Postagram" says "Admiralty Whitehall" and has a picture but is a very poor reproduction.
