Can you post just the bit with 'camp or hospital' as large as possible or is that the only image you have ?
Hi Steve Is it possible to just photo the camp section, the top seems OK to read but as it was all written probably in pencil its starting to fade and if you could provide a close up of that section specifically it would I think help Beat me to it TD

In that case can I suggest either asking CEPOW for a better copy/image or obtaining an original copy for yourself as I think the only way you are going to be able to answer your questions is to have a clearer image Maybe contact this member who has this web site and knows about liberation questionnaires - Archive Research POW Liberation Questionnaires TD
Doubt if COFEPOW have better image as the LQs were photographed at Kew by volunteers. As said previously many were completed in pencil. There may be someone (not me!) with advanced photo enhancing skills. Another thought is to search for a possible LQ for the Camp Leaders, which are legible, and see if their LQ is more legible for the relevant dates. Death Railway Tim
I think the list of camps continues overleaf, but Cofepow only publish the first page. I think this chap makes an excellent witness: the form was very badly designed based on MI9's experience with European prisoners of war, hence all the questions about escapes. The original will have to be seen, also the WO347 hospital record, but Kew will have to open again for that, but the TBR may have more. There is a WO356 card and the photo above is from the BRE RCOS Pilot roll which mentions return to River Valley Road. He finishes at Chungkai in May 44 and goes to River Valley Road so did he end up on a ship? I am not sure what "X2" is in the above from the Pilot Roll ( WO361/2190) I make the list on the questionnaire: Buller Camp with Major Carslaw RA (1942) Farrar Road April to June (1942) Towner Road June to October (1942) [unreadable}( part of/ administered by ) Changi October 42 to February 43 with Captain Renton, 3 Corps Signals {unreadable} ( part of/ administered by ) Changi February to March 43 Kanyo Thailand March to June 1943 with Major March{?} RASC and Captain Turford FMSF {FMSVF?} Tasao Thailand June to August 1943 with Lt/Col Knights R.Sigs Kanburi Thailand August to November 43 with Lt/Col Knights R.Sigs Chungkai Thailand November 43 to May 1944 with Lt/Col Outram I will check the WO356 card. The card does cross-reference to Japanese, two of whom were prosecuted. JC/144 is Towner Road Singapore cross/reference Maharra Sgt also Farrar Park ? Not prosecuted JC/42 is Kanyu cross/reference Usuki, Kishio "Kanyu Kid" ( prosecuted War crimes file WO235/918) JC/51 is River Valley Road cross/reference Sazuki: "The Pig", "Albert", "Mussolini" not prosecuted JC/3 is Kranji No.2 Working Camp Mandai Village cross/reference Yoshikawa, Taira Sgt/Maj and Takahashi 2* Class ( Yoshikawa was prosecuted War crimes file WO235/841) I can't find that he swore an affidavit.
X2 may refer to the 'X' Parties who were employed building tunnels and bunkers for the Japanese in expectation of an Allied attack. Funk Hole Construction Tim
From WO 345/41 here is his Japanese Index Card: and the reverse: Note the release date of 20/11/2 (2 Nov 45) is the date Changi Camp closed rather than Stanley's actual release date. In all probability he could well have been back in the UK by this date. Tim
