I believe this is Polish issue - the number format ties in with some Polish issue paybooks I have and I think the AP is Army Poland or the Polish equivalent.
It may well be that Henryk Kaus is the Polish equivalent of John Smith i.e. a very common name however an internet search brings up a number of results, for instance:
