I understand that Army Form B.194E was used for "adverse reports" for officers, but does anyone know examples of the circumstances under which such a form would be used? Would it be used in circumstances where the officer was considered unsuitable for command, or would a specific disciplinary issue trigger the use of the form? Or both? Any information would be much appreciated.
